Casters
Get all four spell fragments. There's a full page, a torn full page, a right half of a page, and a left half of a page. The fragments can be Clockwork, Ancient Parchments, or Spell Fragments.
Next, kill one of the bosses in inner prison and loot an Ancient Scroll Kit.
If your fragments aren't all regular spell fragments, go to the entrance of outer prison (the succor point). Find A Vaporous Projection (Prison of Admyrrza •) and give him your non-regular spell fragments. He should return to you a regular spell fragment for each of them.
Talk to A Vaporous Projection (Prison of Admyrrza •) at the entrance of outer prison (the succor point) with all the proper pages and he should give you a stone.
Finally, place each of your spell fragments and the stone you get from A Vaporous Projection (Prison of Admyrrza •) into the Ancient Scroll Kit and hit combine.
Priests
Obtain all four spell fragments. There's a full page, a fragment that looks like zombie flesh, a right half of a page, and a left half of a page. The fragments can be Clockwork, Ancient Parchments, Spell Fragments, or Timeless.
Once all four fragments are obtained seek out A Vaporous Projection (Prison of Admyrrza •) at the zone-in (succor point) in Prison of Admyrrza. Do not hand A Vaporous Projection (Prison of Admyrrza •) your pages, instead Hail A Vaporous Projection (Prison of Admyrrza •) and it will give you a Cursed Stone. This may not work if you still have legacy pages (old pages from before a patch that fixed them) -- if you have these legacy pages hand them to A Vaporous Projection (Prison of Admyrrza •) to receive new pages.
Travel to Thazeran's Tower and obtain a Tablet of the Four; it's a ground spawn. You can find it by casting invisibility and heading into the room on the right from the zone-in. It will be in a small room with a stone monument just off of the large room with the red portal looking thing in the center.
Next, go to the Southern Newport Caster Guild (near the Bank and the Mansion of Portals). Hand the Tablet of the Four to Del Jarels (the Magician Guildmaster). Go through his quest dialogue and make sure you have room to receive a bag.
You receive Elemental Attuned Sealer from Del Jarels.
Lastly, place the 4 fragments and the Cursed Stone you received from A Vaporous Projection (Prison of Admyrrza •) into the Elemental Attuned Sealer and hit combine.
